I just want to confirm that I have all the right forms to complete my application to adjust status and apply for residence here:
I 130 - Petition for Alien Residence
G 325 - Biographic Information (me and him)
I 765 -Application for Employment Authorisation
I 485 -Application to register permanent residence
I 864 -Affidavit of Support.
I'd be really appreciative if anyone could confirm that this is correct or let me know if I've missed anything! Also I am slightly confused about the medical information -do i need to submit a form for that as well?

Before you submit the I-485, you need to get a medical exam done by an approved doctor (which you can find on the USCIS.gov website). It is form I-693 and can be downloaded here:

The medical exam form that the I-485 talks about is the I-693 as well as the results (which the doctor will give to you in a sealed envelope).
Set up an appointment as soon as you can. You will have to bring the I-693 with you to the appointment and the doctor will give you a physical. They mainly check for anything that you might have that could be contagious like Tuberculosis and HIV as well as your mental health. It helps to bring any vaccination records that you have, but they are not required.
After couple of days after the exam is completed, the doctor will give you a large, sealed manila envelope that you have to send in with your I-485. You also have the pay for the exam out of pocket and for my wife it was about $260 a year ago.
Also, there are different Biographic Information forms. They all look the same, but make sure that you have the G-325A like it states in the instructions for the I-485 and not just the G-325 or the G-325B.

My wife and I did it all ourselves. EVERY question that we had involving our forms were answered by calling up the USCIS. My advice about getting an attorney, fill out the forms as much as you can then call the USCIS for the rest. If your not confident after that, get an attorney.

You have pretty much got it covered....however, you may also want to include a I-131 for advance parole if your going to need to leave the country...you can only file that however if you havent accumulated ';bad time'; by overstaying your visa.
